The Inframammary Fold Approach
The inframammary fold—the natural crease at the base of the breast—is the classic and most common method for breast augmentation, not only worldwide but also in our Portland practice with Dr. Connall at Esprit® Cosmetic Surgeons. This approach involves a small, approximately 1.5-inch incision. Although the resulting scar is typically visible upon close inspection, it fades beautifully and usually blends into the natural crease, especially in patients with deeper folds or mildly droopy breasts. Many surgeons rely solely on this method simply because it’s straightforward and proven, while only a few—like Dr. Connall—can confidently perform the more technically demanding axillary endoscopic “scarless” technique.
Special Considerations for Patients Without Natural Folds
For Portland patients without a well-defined crease, hiding the inframammary scar can be more challenging at first. In most cases, though, a natural fold or a significant downward arc of the breast will develop after augmentation, helping to conceal the scar from direct view. However, if you prefer not to have any scar on your breast, our “scarless breast augmentation” with the axillary approach is an excellent alternative. In Japan and China axillary augmentation is the more common approach to breast augmentation.

Women, especially those with deep natural breast folds, frequently choose this reliable method because the resulting scar blends smoothly into the crease. Silicone implants are placed effortlessly through the inframammary incision. Once the implants are positioned, we assess size, symmetry, and overall shape. To ensure the most natural result, we often have you sit up so that we can evaluate the outcome in a real-world posture. Finally, the incisions are meticulously closed with dissolvable sutures beneath the skin, covered with surgical tape, and supported by a post-surgical sports bra to promote optimal healing and enhance your new contour.

Recovery Experience
Recovery from breast augmentation at Esprit® Cosmetic Surgeons is designed to be swift and comfortable, no matter which method you choose. Whether you opt for the classic inframammary fold (IMF) approach or our advanced, scarless axillary endoscopic technique, the post-operative experience is remarkably similar. Most of the post-surgical discomfort comes from creating the implant pocket—manipulating the muscle and tissues—a necessary step in every procedure. When implants are placed above the muscle or in a subfascial pocket, recovery is even more streamlined, resulting in less disruption and a faster return to normal activities.
After surgery, you will not deal with drains, pain pumps, or bulky chest wraps. Instead, you simply wear a supportive post-op bra, much like a sports bra, which gently holds everything in place as you heal. You can typically head home just one hour after your procedure to rest and recuperate, and by the next day, most patients feel well enough to shower, wash their hair, and resume light daily activities. Many of our patients comfortably return to home and work activities within 2 to 3 days.
Guided by Dr. Connall’s gentle technique, our recovery protocols are rapid, and patient focused. Approximately two weeks after surgery, you can start incorporating light exercises, and within four weeks, most patients are ready to resume full activities.
